All patients including those whom are covered by insurance are responsible for the minimum standard fee at the time of service.  An additional 25% service fee will be added for unpaid bills at the time of service.  Insurance patients; once deductable has been met and the payer is sending reimbursement the patient will then only pay the determined co-pay amount. All unpaid claims will be the full responsibility of the insured party.

Ascent Health Center is an out of network provider for ALL insurance carriers.  A quote of benefits is then relayed from Ascent Health Center to the patient as information only not a guarantee of payment.

Reimbursement is subject to the terms and eligibility of the policy at the time of service. Claim submission does not guarantee payment and patients will be financially responsible for remaining unpaid balance.

Any checks you may receive from your insurance company are the property of Ascent Health Center.  All insurance checks should be sent to us from your insurance company; however, if you do mistakenly receive a check from your insurance company you need to sign the check over to Ascent Health Center.
